*ORDERING IN PERSON/ We charge $40 up front for a 20 min consultation and if an order is placed within 2 wks, the fee is deducted from the cost of the wig.

• Virtual reality makeover featuring short, medium, and long hair, lipstick, eye shadow, blush, foundation, jewelry, eye wear, etc
• Attachment service/ Full lace wig will be attached with temporary "Lace-fix Tape" for 1-10 days. Client must style hair the way they would like to wear it under the wig. For a more permanent attachment, purchase our adhesive from our product line.

CANCER and ALOPECIA PATIENTS/Getting Your Health Insurance Provider to cover the cost of the Lace Wig
Note: We are ot a Medical or Health Care Provider. WE DO NOT process Medical Claims Nor Do We accept prescriptions.

When you lose your hair due to (pix)medical reasons such as chemotherapy, alopecia,trichotillomania,,etc., request a prescription from your phisician for a "FULL CRANIAL PROSTHESIS." This will enable the Full Lace Wig to be covered by most meical insurance companies; and if they don't cover all of it, they will cover p to 80% of the cost. You must contact your insurance company directly to see what their procedure is to obtain full or partial coverage for the "FULL CRANIAL PROSTHESIS" which may also be a Tax Deductable Medical Expense.

STEPS to FOLLOW

• Call your Health Insurance Provider and ask them to check the coverage of your policy for a "FULL CRANIAL PROSTHESIS" for medical hair loss.
• If it is covered, ask them to expain their procedure for obtaining payment for a "FULL CRANIAL PROSTHESIS"
• Some insurance companies will require you to pay upfrontand then be re-imbursed, while others will pay directly upfront.
• When doing a custom order, inform us that it is a prescription for a "FULL CRANIAL PROSTHESIS" to ensure that your invoice will satisfy your insurane company.
• Complete all insurance forms and make sure they are signed by your physician.
• Ask your physician to also write a letter explaining your hair loss and its emotional effects on you and to state that it is not just for cosmetic reasons for the order.
• Keep copies of all documentation and invoices for tax exempt purposes.
• Follow the instructios given to you by your health insuance provider for submission of the prescription so that you will obtain payment for the"FULL CRANIAL PROSTHESIS."

APPLYING for a GRANT or VOUCHER

Tha National Alopecia Areata Foundation(NAAF) has established the Ascot Fund which is a special pre-purchase program providing financial assistance toward the purchase of a Lace Wig to individuals who have been diagnosed with Alopecia Areata and are struggling financially. We are also in the process of developing a foundation to assist in this arena. To find out if you qualify, contact them @:National Alopecia Areata Foundation

The American Cancer Society also has a program available to assist those who qualify with the purchase of a Lace Wig. If your insurance does not cover the full cost of the uit, you may qualify for an allotment. They will give you a voucher of up to $75.00 to help cover the cost of the unit. You must apply before you custom order your wig from us. Contact the American Cancer Society @ (800) 227-2345. Ask for the person responsible for financial assistance toward the purchase of a wig.
